Minnesota offers several health care programs to people who qualify. Health care programs pay for medical care for very low-income people. Eligibility for the program(s) is based on income and assets.

To obtain an application for medical programs, contact Sibley County Human Services or you may download an application below.  When you apply, you will be required to fill out an application and you will be asked for proof of the information you provide.
